I agree that it would be wonderful to get at the issues you describe ... those below the waterline... aber der frager ist. What about the issues ABOVE the waterline... we should not chase what we cannot see until we understand that which is staring us in the face. Let us learn to document explicit knowledge and not lose the knowledge through attrition, etc ... we don't even know how we make our products and deliver out services ... trust me.
