Most of the companies leading the KM charge have moved on from architecture and technology challenges to working with people issues. You will find many examples in "The knowledge management fieldbook" by Wendi Bukowitz & Ruth Williams, 1999. Carla O'Dell & Jackson Grayson "If we only knew what we know", 1998 also supply useful case studies. What is not clear from your post is if you are looking for case studies or for culture directed KM strategies. IF the latter, I suggest you get a copy of Jim Botkin's Smart Business, "How knowledge communities can revolutionize your company" 1999. Some of the culture issues are trust, deep dialog, inquiry, reflection, prototyping, mentoring. Literature on communities of practice, systems thinking, knowledge sharing, learning communities is where you wish to look. The comapny that is often cited as having an excellent knowledge sharing system is Buckman labs. I'm not sure even they would qualify as having solved tacit knowledge use or transfer.
