Hello
First, nothing exists, at least as a single entity, that meets the needs you have defined.
Secondly, the requirement as expressed is visionary, strategically desirable yet operationally (virtually) impossible right now ... unless you have less than 10 people working in your company.
At best, and in this context, your IM plan will be a high level, holistic and visionary plan which should drive IM / KM strategies and subsequent projects.
