Interesting discussion here. Somehow I seem to think any team or project that needs or profits from a separate final debrief has already missed the opportunity. The dialog should have been sufficiently abrasive and the communication strong enough to surface the learnings while the action was underway. I can appreciate in some cases this is just not possible e.g. in the heat of battle, but lack of time to reflect, reservation of feedback till all is over are mild pathologies in their own right. If the KM was functioning and efficient should there even be a debrief?
