A nice thread you and Don have here. A live issue with us is "who is the CKO in the small-medium enterprise?"Your real perception that CKOs will need to step out of their professional comfort zone if they are to maximise their impact is absolutely valid, but for the SME the real problem is one of resources. They can't afford a good level CKO (yes, I know they can't afford NOT to have one, but that issue only plays when they are further down the line!) and another high level operating exec. will not have the time to devote to real strategic leverage.
Its a chicken/egg situation but we have to solve it. We are building one solution here in the UK by offering SMEs entry to small groups (say 6 organisations in each) led by one of our people as consultant CKO. Its cost-effective but we have to work very hard to make it fully workable, and in fact our success will be shown when members get to be able to appoint their own and can leave the group. Another paradox Don!
