May I suggest you get hold of "Continuity management. Preserving corporate knowledge and productivity when employees leave", by Beazley, Boenisch & Harden, Wiley 2002. Chapters 8-11 provide a useful template, questions and describe a formal process for 'knowledge harvesting' (not a term I favor).
This is one of the basic activities within knowledge mapping
