We have done several sets of knowledge maps for clients that documents the knowledge flow, and the decisioning that occurs to utilize that knowledge in a work environment.
One thing to keep in mind here is the difference between information and knowledge. Collecting information about how something occurs does not confer knowledge--only information. Knowledge-based information systems collect information that appears to work if replicated. However, despite the information collection, it might not work exactly the same way it did for the original provider of the information. That difference is in the tacit knowledge retained and not expressed by the individual--perhaps not even consciously understood by the individual--that allows him/her to perform the same task on a recurring basis.
