Hi Mark,
Wonder if you are familar with communities of practice and pattern languages. This seems an ideal opportunity to try a pilot in both these areas.
Communities of practice or knowledge communties will give your firm a social environment for sharing insights, swapping stories and for capturing experience. What is key here is sharing distinctions, understanding the role of heuristics and mining patterns. I would suggest you concentrate on conversations and enabling discussions rather than capturing data. Here are some notes & links I hope will help. A key text in this area is Wenger, McDermott & Snyder's recent book (2002) "Cultivating Communities of Practice"
