Don't forget that although people leave the organisation (via the various ways above), there are also people coming IN to the organisation who, while they're bringing in fresh perspectives/ideas/knowledge, need to establish networks within the organisation to learn and contribute informally. Effective KM tools can help here by assisting people to make contact with appropriate individuals and groups within the organisation. This is all about linking people and pointing to relevant info rather than trying to capture it.
