Freeform webpages with a top notch search engine and some meta-data tagging is what has worked best for me. Straight database applications have all languished for want of interest and attention. The key is having interesting contact points so you can support informal personal networking rather than crisp competencies that you can select for from a fixed field DB.
Yellowpages that survive have been driven by self-motivated interest, by top management asking sharp questions and by 'publishing' stories on how the entries helped find resources and people for critical projects.
I'm not aware of any single source for the definitive article on yellowpages. Recently there have been a spate of new sofware applications to address this niche KM market. You may find this Davenport brief a useful read.
Sopheon has launched their Intota product in this area
At the risk of over-emphasis, let me repeat, the key here IMO is sharing personal networks, it is the tacit relationships rather than individual competencies that carry the real value.
